Our mission at Great Lakes Endodontics is to make each patient's experience pleasant, comfortable, and of the highest quality.  Root canal therapy is a safe, effective way to eliminate the cause of the pain you may be experiencing, keep your natural tooth, and maintain your dental health.


First Visit

If your family dentist feels you need endodontic treatment, he or she may refer you to our office.  Continue to take your medications as directed.  When you arrive, our staff will assist you in completing our registration form.  You may prefer to provide that information to us in advance of your appointment by filling out the online Patient Registration form here or by clicking the button below. 

Your consultation will include a review of your health history as well as an oral evaluation that includes several diagnostic tests to help us determine your treatment needs.  With advance planning most endodontic treatment can be completed the same day as the consultation; however, a complex treatment plan may require a second appointment.

Please assist us by providing the following information at the time of your consultation:

  • Referral slip from family dentist and any digital images if applicable
  • List of medications you are presently taking
  •  If you are pregnant, a letter from your obstetrician
  • If you have dental insurance, your insurance card
  • Your registration form if you prefer to print and complete it at home

IMPORTANT: All patients under the age of 18 years of age must be accompanied by a parent or guardian at the visit.
